Immunizations by age group

Learn what immunizations are needed by age (birth to 16 years old) and for childcare, early childhood programs and school in Minnesota.

Birth - 3 years

Immunizations required for early childhood programs and childcare:

  • Hepatitis (Hep A) - 2 doses.
  • Hepatitis (Hep B) - 3 doses
  • DTaP/DT - 4 doses.
  • Polio - 3 doses.
  • MMR - 1 dose.
  • Hib - 3 or 4 doses, final dose determined by the age of previous dose received.
  • Pneumococcal - 3 or 4 doses, final dose determined by the age of previous dose received.
  • Varicella - 1 dose.
  • Rotavirus - 2 or 3 doses depending on formula.

Immunizations that are recommended but not required:

  • COVID-19 - updated vaccine.
  • Influenza - annually for all children age 6 months and older.
  • RSV (age 0 - 8 months) – 1 dose depending on maternal RSV vaccination status.

4 - 11 years

Immunizations required for kindergarten (4 - 5 years old) to 6th grade:

  • Hepatitis (Hep B) - 3 doses.
  • DTaP/DT - 4 - 5 doses, final dose determined by the age of previous dose received. 
  • Polio -  3 or 4 doses, final dose determined by the age of previous dose received.
  • MMR - 2 doses.
  • Varicella - 2 doses.

Immunizations that are recommended but not required:

  • COVID-19 - updated vaccine.
  • Influenza - annually for all children age 6 months and older.
  • Human papillomavirus (HPV) - ages 9 - 12 years old.

Immunizations required for​ 7th to 12th grade:

  • Tdap - 1 dose.
  • Meningococcal (MenACWY) and booster - 2 doses.

Immunizations that are recommended but not required:

  • Human papillomavirus (HPV) - ages 9 - 12 years old.
  • COVID-19 - updated vaccine.
  • Influenza - annually for all children age 6 months and older.
